Dr. Ahsan Jafir Named Rounding Cardiologist for Vierra Communities’ VIVID Cardiac Program at Vierra Falls Church and Admiral Health & Rehab

Renowned Fairfax County interventional cardiologist joins the VIVID Cardiac program, bringing more than a decade of complex cardiac care to residents during the highest-risk 30 days after a cardiac event.

Vierra Communities announced today that Dr. Ahsan Q. Jafir, DO, a board-certified cardiologist and interventional cardiologist, has been named rounding cardiologist for its VIVID Cardiac program at both Vierra Falls Church and Admiral Health & Rehab. Dr. Jafir now stands at the center of the program, providing direct, hands-on cardiology leadership at the bedside throughout the 30-day post-discharge window that carries the highest risk of cardiac complications and hospital readmission.

Dr. Jafir brings more than a decade of experience treating complex cardiac conditions. He currently practices with INOVA Medical Group – Cardiology in Manassas and Fairfax, Virginia. He spent more than 13 years as an interventional cardiologist with UVA Community Health Cardiology (formerly Novant Health UVA Health System Prince William Cardiology) in Manassas, and he is board-certified in interventional cardiology, cardiology, and nuclear cardiology. His fellowship training includes an interventional cardiology fellowship at Winthrop University Hospital, State University of New York, and a cardiology fellowship at the University of Medicine and Dentistry of New Jersey, where he served as chief cardiology fellow.

As rounding cardiologist, Dr. Jafir manages complex cardiovascular conditions directly rather than reviewing charts from a distance. He works daily alongside therapy, nursing, and medical teams at both communities. His hands-on evaluations help residents move through recovery more safely and return home with confidence.

“Too many patients leave the hospital and fall through the cracks in that first month — that’s where complications and readmissions happen. Being on the ground with the therapy and nursing teams every day, not just on call, is what actually changes outcomes,” said Dr. Jafir.

A Program Built to Beat the Odds

VIVID Cardiac is Vierra Communities’ specialized post-acute cardiac recovery program, engineered to close the gap between hospital discharge and full heart health, built around each resident’s diagnosis rather than a generic protocol. The program delivers 24/7 cardiac nursing oversight and has driven a 30% average reduction in cardiac-related readmissions. Its capabilities include weekly heart failure rounds, cardiac consultation available 24/7 by phone or telehealth, in-house EKGs and echocardiograms, 24/7 diagnostic and laboratory capabilities, IV therapy including IV Lasix push, in-house 48-hour Holter monitoring, medication management, heart-healthy nutrition, chronic condition management, respiratory therapy, and 7-day-a-week PT, OT, and speech therapy services.

The program manages a broad range of high-acuity cardiac conditions, including heart failure (CHF), post-CABG recovery, myocardial infarction, post-TAVR and valve repair, atrial fibrillation, cardiomyopathy, pacemaker and ICD placement, and peripheral artery disease. Every resident’s stay begins with a cardiologist evaluation on admission and an individualized care plan, continues with onsite provider presence that helps avoid unnecessary hospital transfers, and closes with a full discharge and transition planning package: follow-up scheduling, home health coordination, and medication management for the resident and their family.

“We don’t hand patients a pamphlet and wish them luck. Our cardiologists remain engaged throughout the stay, adjusting, watching, and advocating for each patient’s best possible outcome,” said Dr. Tavneer Malik, cardiologist with the VIVID Cardiac program.
